Summary

Isolated ocular lichen planus is a rare autoimmune condition causing severe conjunctival scarring. Early diagnosis and treatment with immunosuppressants are crucial to preserve visual function.

Background:

  • Lichen planus is a common autoimmune condition affecting skin and mucous membranes.
  • Isolated ocular lichen planus is an extremely rare presentation.
  • It often involves eyelids, conjunctiva, and cornea, leading to severe scarring.

Observation:

  • A 79-year-old man presented with chronic, treatment-refractory keratoconjunctivitis.
  • Examination revealed bilateral conjunctival hyperemia, subepithelial fibrosis, and symblepharon.
  • Histopathology showed acanthosis, basement membrane thickening, and mononuclear infiltrate.

Findings:

  • Direct immunofluorescence indicated linear fibrinogen deposition, suggestive of lichen planus.
  • Ultrastructural analysis revealed significant basement membrane abnormalities.
