A free search approach improved interjudge reliability for rating aspiration and penetration during videofluoroscopic swallow (VFS) studies. This unguided observation method offers better accuracy than directed checklists for specific VFS variables.

Background:

  • Clinician reliability in rating videofluoroscopic swallow (VFS) variables is often poor.
  • Current VFS reliability studies may use methods that lead to unnatural judgments.

Purpose of the Study:

  • To compare the interjudge reliability of directed (checklist) versus free search (unguided observation) approaches for rating VFS swallow studies.
  • To determine which VFS rating method enhances diagnostic accuracy.

Main Methods:

  • Ten speech pathologists were divided into directed search (checklist) and free search (unguided) groups.
  • Judges interpreted 20 VFS examinations, rating dysphagia severity, affected stages, symptoms, and attributes.
  • Interjudge reliability was compared between the two search methods.

Main Results:

  • Free search yielded significantly better interjudge reliability for rating aspiration and penetration (substantial to almost perfect agreement) compared to directed search (moderate agreement).
  • Reliability for dysphagia severity ratings was comparable between both methods (moderate to almost perfect agreement).
  • Reliability for other dysphagia symptoms and attributes varied and did not differ significantly between groups.

Conclusions:

  • An unguided, free search approach enhances interjudge reliability for specific videofluoroscopic swallow study findings, particularly aspiration and penetration.
  • Clinicians may benefit from unguided observation for improved diagnostic consistency in VFS assessments.
  • The choice of search method impacts reliability in VFS interpretation.
