Purpose of the Study:

  • To identify factors that impede the early detection of Chronic Obstructive Pulmonary Disease (COPD) in elderly individuals residing in rural Japan.

Main Methods:

  • A cross-sectional study compared elderly individuals with COPD (n=60) to healthy controls (n=60), matched for age, sex, height, and smoking history.
  • Respiratory function (spirometry, FEV1/FVC ratio), physical function (grip strength, respiratory pressures, toe grasp), walking ability, and quality of life (QOL) were assessed.

Main Results:

  • Individuals with airway obstruction (COPD) exhibited significantly inferior physical function and instantaneous walking ability compared to controls.
  • Walking endurance (6-minute walk test) and quality of life (QOL) were comparable between groups, failing to differentiate COPD patients.

Conclusions:

  • Well-preserved walking endurance, QOL, and proximal muscle strength in COPD patients can mask the disease, reducing the motivation to seek medical care and hindering early detection.
  • Spirometry is a more practical screening tool than physical function tests for early COPD detection and staging in primary healthcare settings for the elderly.
