Summary

Z-palatopharyngoplasty plus radiofrequency of the base of the tongue for obstructive sleep apnea/hypopnea syndrome can cause temporary changes in middle ear pressure. These effects were not significant after 3 months, indicating no permanent otologic issues.

Background:

  • Obstructive sleep apnea/hypopnea syndrome (OSAHS) is often treated with surgical interventions.
  • Z-palatopharyngoplasty combined with radiofrequency of the base of the tongue is a surgical option for OSAHS.
  • The impact of this combined surgical approach on middle ear function requires investigation.

Purpose of the Study:

  • To evaluate the effect of Z-palatopharyngoplasty plus radiofrequency of the base of the tongue on middle ear pressure and function.
  • To assess the duration and significance of any observed changes in middle ear function post-surgery.

Main Methods:

  • Retrospective review of a prospective data set from a tertiary care center.
  • Inclusion of 47 patients with OSAHS who underwent the specified surgical procedure, with no prior ear disease.
  • Preoperative and serial postoperative (3 days, 7 days, 1 month, 3 months) pure-tone audiometry and tympanometry were performed.

Main Results:

  • 26% of patients reported temporary otologic symptoms (ear pressure, otalgia) within the first week post-surgery.
  • A transient trend toward reduced middle ear pressure was observed, most noticeable by day 3.
  • No permanent otologic discomfort or significant middle ear pressure changes were noted by 1 week or 3 months post-surgery.

Conclusions:

  • Z-palatopharyngoplasty plus radiofrequency of the base of the tongue for OSAHS can induce temporary alterations in middle ear function.
  • These middle ear function changes are transient and resolve by 3 months post-procedure.
  • The surgical intervention does not appear to cause lasting negative effects on middle ear pressure or function.
