Area of Science:

  • Hepatobiliary Surgery
  • Minimally Invasive Surgical Techniques
  • Surgical Oncology

Background:

  • Pure laparoscopic liver resection offers cosmetic benefits but is limited by technical demands.
  • A need exists for less invasive liver resection techniques that are more widely applicable.

Purpose of the Study:

  • To introduce and describe a small skin incision left hepatectomy technique.
  • To evaluate its feasibility, safety, and cosmetic outcomes compared to standard methods.

Main Methods:

  • A small (8-10 cm) right subcostal incision with an Alexis wound retractor.
  • Utilizing Arantius's ligament traction for hepatic vein access.
  • Employing the liver-hanging maneuver for safe parenchymal transection under direct vision.

Main Results:

  • The technique allows safe manipulation of the hepatic hilum and liver transection.
  • It achieves satisfactory cosmetic results comparable to standard hepatectomy.
  • It does not increase risks associated with pure laparoscopic hepatectomy.

Conclusions:

  • The small skin incision left hepatectomy is a feasible and safe alternative.
  • It is suitable for experienced liver surgeons seeking minimally invasive options.
  • This technique can be readily adopted by centers not performing pure laparoscopic surgery.
