Summary

Anchoring decubital ulcers with gluteal rotation flaps, using a deepithelialized tip for subcutaneous support, promotes stable healing. This technique prevents recurrence in paraplegic patients, allowing early mobilization and undisturbed wound healing.

Area of Science:

  • Plastic Surgery
  • Orthopedic Surgery
  • Wound Healing

Background:

  • Flap stability is crucial in decubital surgery to prevent shearing forces.
  • Early mobilization and primary wound healing depend on secure flap anchoring.
  • Sacral decubital ulcers pose a significant challenge in paraplegic patients.

Purpose of the Study:

  • To evaluate the efficacy of gluteal rotation flaps with a deepithelialized tip for anchoring sacral decubital ulcers.
  • To assess initial wound healing and long-term recurrence rates after this surgical technique.

Main Methods:

  • A uniform group of eight paraplegic patients with sacral decubital ulcers were treated.
  • Gluteal rotation flaps with a deepithelialized tip were used for subcutaneous anchoring on the contralateral ischial tuber.

Published on: June 8, 2013

  • Initial wound healing and recurrence after one year were evaluated.
  • Main Results:

    • Uneventful wound healing was observed in all but one patient.
    • No signs of recurrence or instability were noted in any of the flaps after one year.
    • The deepithelialized flap tip facilitated integration and stabilization.

    Conclusions:

    • Sufficient anchoring of sacral rotation flaps using a deepithelialized part is effective.
    • This technique ensures flap stability, promotes healing, and prevents recurrence in decubital ulcer surgery.
    • Early patient mobilization is facilitated by secure flap anchoring.
