Summary

An education program significantly reduced prostate contouring variations for radiation oncologists using CT and MRI. This training improved both inter- and intraobserver consistency, particularly with MRI imaging.

Area of Science:

  • Medical imaging and radiation oncology.
  • Radiotherapy planning and target delineation.

Background:

  • Accurate prostate contouring on CT and MRI is crucial for effective radiation therapy.
  • Inter- and intraobserver variability in contouring can impact treatment outcomes.

Purpose of the Study:

  • To evaluate the effectiveness of an educational program in reducing prostate contouring variations.
  • To assess the impact of the program on both inter- and intraobserver variability among radiation oncologists.

Main Methods:

  • Five radiation oncologists contoured prostate datasets on CT and MRI.
  • Contouring was performed before and after an educational intervention.
  • Observer variation was quantified using the volume ratio (VR).

Main Results:

  • Post-education, mean interobserver VR decreased by 15% for CT and 40% for MRI.
  • Mean intraobserver VR decreased by 9% for CT and 16% for MRI.
  • The educational program demonstrated a greater impact on MRI contouring variation.

Conclusions:

  • A structured education program effectively reduces prostate contouring variations.
  • The benefits were more pronounced for MRI compared to CT.
  • Continuing medical education in target contouring is recommended for radiation oncologists.
