Obedience01:08

Obedience

According to obedience research, we may harm others under the forceful pressures of an authority figure (Milgram, 1974). How about if the inappropriate orders were delivered with less force? The increasing interdependence between nurses and physicians compelled Hofling and his colleagues to explore nurses’ reactions to a potentially harmful medical request made by the perceived authority figure, the doctor (Hofling, Brotzman, Dalrymple, Graves, & Pierce, 1966). In this situation, obedience...

Many orthopaedic departments do not follow their own published clinical recommendations. This study highlights a gap between published research and actual practice in the Journal of Bone and Joint Surgery (British edition).

Area of Science:

  • Orthopaedic Surgery
  • Medical Publishing
  • Clinical Practice Guidelines

Background:

  • The Journal of Bone and Joint Surgery (British edition) [JBJS (Br)] is a leading orthopaedic journal.
  • Concerns were raised about the implementation of clinical recommendations published in JBJS (Br).
  • There is a need to assess adherence to published clinical recommendations by originating departments.

Purpose of the Study:

  • To determine the extent to which hospitals follow their own published clinical recommendations.
  • To investigate the gap between published orthopaedic research and clinical practice.

Main Methods:

  • A systematic review of articles published in JBJS (Br) between January 2005 and December 2006 was conducted.
  • Articles recommending a change in practice were identified.

Related Experiment Videos

  • The originating departments were contacted to assess implementation of the recommendations.
  • Main Results:

    • Seventy-nine papers from 87 hospitals across 22 countries were analyzed.
    • Published clinical recommendations were followed by only 56% of institutions.
    • A significant proportion (42%) of institutions did not implement their own published recommendations.

    Conclusions:

    • A substantial number of institutions do not adhere to their published clinical recommendations.
    • This suggests a potential flaw in the dissemination and implementation of clinical research findings.
    • There is a discrepancy between publishing clinical recommendations and practicing them.
