Purpose of the Study:

  • To investigate the association between lung function impairment and COPD hospitalizations.
  • To examine the relationship between COPD hospitalizations and subsequent mortality.

Main Methods:

  • A population-based sample of 20,571 participants with 10-year median follow-up was analyzed.
  • Participants were classified using modified Global Initiative on Obstructive Lung Disease (GOLD) criteria based on prebronchodilator lung function.
  • COPD hospitalizations were identified via ICD-9 codes (490-496); adjusted incidence rate ratios (IRR) and hazard ratios (HR) were calculated.

Main Results:

  • Increased risk of COPD hospitalization was observed with each COPD stage compared to normal lung function, with the highest risk in GOLD stages 3 or 4 (IRR 25.5).
  • COPD hospitalization significantly increased the risk of subsequent mortality (HR 2.7), even after controlling for severity and prior hospitalizations.
  • The increased mortality risk associated with hospitalization was consistent across all modified GOLD stages.

Conclusions:

  • COPD severity correlates with a higher incidence of severe exacerbations requiring hospitalization.
  • Severe exacerbations, irrespective of COPD stage, are linked to increased short-term and long-term all-cause mortality.
