Heroin use via "chasing the dragon" can rarely cause delayed-onset toxic leukoencephalopathy. This rare neurological disorder affects brain white matter, leading to severe symptoms but can resolve with supportive care.

  • This method is rarely associated with delayed-onset spongiform leukoencephalopathy, a serious neurological condition.

Observation:

  • A 46-year-old woman with heroin dependence presented with altered mental status.
  • She later developed catatonia, abulia, and akinetic mutism.
  • Brain MRI revealed bilateral symmetric white matter lesions consistent with toxic leukoencephalopathy.

Findings:

  • The patient's neurobehavioral symptoms and MRI findings were consistent with toxic leukoencephalopathy.
  • Her condition improved with supportive treatment over a 2-month hospital stay.

Implications:

  • Acute neurobehavioral changes in individuals with heroin exposure warrant consideration of toxic leukoencephalopathy.
  • Early recognition and supportive care may lead to recovery from this rare heroin-induced neurological complication.
