Open Angle Glaucoma: Treatment01:27

Open Angle Glaucoma: Treatment

In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. The treatment for open-angle glaucoma focuses on reducing the elevated intraocular pressure by either decreasing the secretion of aqueous humor or increasing its outflow.

Summary

Titanium prostheses for ossicular reconstruction significantly improved hearing in most patients with chronic otitis media. Partial reconstructions showed higher success rates than total reconstructions, with better outcomes in staged total reconstructions.

Area of Science:

  • Otolaryngology
  • Surgical Innovation
  • Hearing Reconstruction

Background:

  • Chronic otitis media often necessitates surgical intervention for hearing restoration.
  • Ossicular chain reconstruction aims to improve sound conduction through the middle ear.

Purpose of the Study:

  • To review outcomes of ossicular chain reconstruction using titanium prostheses.
  • To evaluate hearing results and complication rates in titanium ossiculoplasty.

Main Methods:

  • Retrospective analysis of 124 ossiculoplasty cases (1999-2008) using Kurz titanium prostheses.
  • Inclusion of partial, total, single-stage, staged, and revision ossicular chain reconstructions.
  • Measurement of pre- and postoperative pure-tone averages, air-bone gap, and extrusion rates.

Main Results:

  • Successful ossiculoplasty achieved in 73.1% of partial and 30.4% of total reconstructions.
  • Mean postoperative air-bone gaps were 16 dB for partial and 26.7 dB for total reconstructions.
  • Five cases of prosthesis extrusion were reported.

Conclusions:

  • Ossicular chain reconstruction with titanium prostheses generally improved hearing status.
  • No significant difference in hearing outcomes between one-stage and two-stage partial reconstructions.
  • Two-stage total ossicular chain reconstruction yielded superior hearing results.
