Insomnia night-to-night variability shows predictable patterns in most patients, challenging the idea that insomnia is always unpredictable. Most individuals with insomnia fall into distinct sleep pattern clusters.
Area of Science:
- Sleep Science
- Chronobiology
- Clinical Psychology
Background:
- Chronic insomnia is often perceived as unpredictable.
- Previous research suggested unpredictability as a key feature of insomnia.
Purpose of the Study:
- To determine if night-to-night variability in insomnia follows predictable patterns.
- To characterize insomnia sleep patterns using objective and clinical data.
Main Methods:
- Prospective observational study of 146 participants with chronic primary insomnia.
- Utilized sleep diaries, questionnaires, and polysomnography (PSG).
- Applied time-series analysis and k-means clustering to identify sleep patterns.
Main Results:
- Identified three distinct insomnia clusters: unpredictable (38%), low probability of insomnia (30%), and high probability of insomnia (49%).
- Clusters differed in age, insomnia severity, mental fatigue, and subjective sleep quality.
- No significant differences were found in objective polysomnography (PSG) sleep variables between clusters.
Conclusions:
- Unpredictability is less prevalent in insomnia than previously suggested.
- The identified clusters may reflect differences in sleep perception and sleep homeostasis dysregulation.
