  • This study aimed to identify key factors associated with bronchiectasis in moderate to severe COPD.

Purpose of the Study:

  • To determine the prevalence of bronchiectasis in patients with moderate to severe COPD.
  • To identify clinical, functional, and microbiological factors associated with bronchiectasis in this cohort.
  • To provide insights into the relationship between COPD severity and co-existing bronchiectasis.

Main Methods:

  • Prospective enrollment of patients with moderate (50% < FEV(1) ≤ 70%) or severe (FEV(1) ≤ 50%) COPD.
  • Data collection included clinical questionnaires, peripheral blood analysis, lung function tests, and sputum microbiology over 6 months.
  • High-resolution computed tomography (CT) scans were utilized for bronchiectasis diagnosis.

Main Results:

  • Out of 92 patients, 53 (57.6%) had bronchiectasis.
  • Independent predictors for bronchiectasis included severe airflow obstruction (OR, 3.87), isolation of a potentially pathogenic microorganism (PPM) (OR, 3.59), and prior hospital admission for COPD exacerbations (OR, 3.07).

Conclusions:

  • Bronchiectasis is highly prevalent in moderate to severe COPD patients.
  • Severe airflow obstruction, detection of PPM in sputum, and a history of COPD exacerbation hospitalizations are significantly associated with bronchiectasis.
  • These findings highlight important clinical correlates of bronchiectasis in COPD.
