Summary

Surgical outcomes for cystosarcoma phyllodes show that while local recurrences are common, they are manageable. However, a significant minority of patients experienced malignant spread, emphasizing the need for careful monitoring.

Area of Science:

  • Oncology
  • Surgical Pathology

Background:

  • Cystosarcoma phyllodes is a rare breast tumor with variable behavior.
  • Understanding optimal surgical management is crucial for patient outcomes.

Purpose of the Study:

  • To review the surgical treatment and outcomes of cystosarcoma phyllodes.
  • To analyze recurrence patterns and survival rates based on surgical approach.

Main Methods:

  • Retrospective review of 31 female patients diagnosed with cystosarcoma phyllodes.
  • Analysis of surgical interventions including excision and mastectomy.
  • Evaluation of local recurrence, distant disease, and survival.

Main Results:

  • Excision (n=20) had a mean tumor size of 4.5 cm; mastectomy (n=11) had a mean size of 15 cm.

Related Experiment Videos

  • Local recurrence occurred in 5/20 excisions and 1/11 mastectomies.
  • Repeat excision controlled recurrences in all treated patients; one mastectomy patient died with disease.
  • Axillary lymph node involvement was rare and associated with advanced disease.
  • Four patients died from malignant spread, challenging the notion of universally benign behavior.
  • Conclusions:

    • Surgical excision is effective for smaller tumors, while mastectomy may be necessary for larger ones.
    • Local recurrences are manageable with further surgery, and do not necessarily indicate systemic spread.
    • Cystosarcoma phyllodes can exhibit malignant behavior with fatal systemic spread, necessitating vigilance.
