Patient-centered care involves delivering care beyond inpatient hospitalization. Reflective practice can enhance a patient-centered approach. Reflective practice is a process of reasoning that considers all aspects of the present situation, including practicalities, learning from personal practice, and consideration of patient needs. Patients appreciate care decisions made while considering their input. Involving the patient in their care provides the patient with a sense of contribution rather...

Area of Science:

  • Psychiatry
  • Adolescent Medicine
  • Medical Psychology

Background:

  • Self-embedding behavior (SEB) is a rare but severe form of self-injury.
  • Adolescents engaging in SEB often have complex psychiatric histories.

Purpose of the Study:

  • To define self-embedding behavior (SEB).
  • To establish a clinical profile of adolescents exhibiting SEB.
  • To highlight the need for rapid, targeted interventions for SEB.

Main Methods:

  • Retrospective analysis of 600 patients undergoing percutaneous soft-tissue foreign body removal.
  • Identification and characterization of adolescents with self-embedded soft-tissue foreign bodies.
  • Data collected included demographics, psychiatric diagnoses, SEB onset, frequency, reasons, object characteristics, and removal techniques.

Main Results:

  • Eleven adolescents (9 female) with SEB were identified, most residing in group homes or psychiatric facilities.
  • Mean age of SEB onset was 16 years, with an average of 1.9 episodes per patient.
  • Suicidal ideation was the most common reported reason for SEB; an average of 2.4 objects, typically metal, were embedded per episode, often in the arm.

Conclusions:

  • Self-embedding behavior (SEB) represents an extreme form of self-injury.
  • Aggressive, timely, interdisciplinary assessment and treatment are essential for SEB.
  • Understanding SEB facilitates prompt intervention to halt self-harm and initiate long-term therapy.
