Background:

  • Hepatic encephalopathy (HE) is a neurological complication of liver disease.
  • The default-mode network (DMN) plays a crucial role in brain function, but its alterations in HE are understudied.
  • Resting-state functional magnetic resonance imaging (fMRI) is a valuable tool for investigating brain connectivity.

Purpose of the Study:

  • To investigate default-mode network (DMN) alterations in patients with hepatic encephalopathy (HE) using resting-state fMRI.
  • To explore the physiological relevance of DMN changes in HE, specifically the correlation with venous blood ammonia levels.

Main Methods:

  • Independent component analysis (ICA) was employed to identify DMN components in 14 HE patients and 14 healthy controls.
  • Resting-state fMRI data was acquired to assess functional connectivity.
  • Statistical analysis compared DMN connectivity between groups and correlated it with blood ammonia levels in HE patients.

  • HE patients exhibited significantly reduced functional connectivity within the DMN, particularly in the right middle frontal gyrus and left posterior cingulate cortex.

  • The correlation between DMN changes in the left angular gyrus and ammonia levels suggests impaired central nervous system organization in HE.
