Bilateral cerebellopontine angle (CPA) lipomas, rare tumors, were found in a man presenting with ear fullness. These CPA lipomas were associated with abnormal hindbrain segmentation, as seen on MRI.

  • Cerebellopontine angle (CPA) lipomas are exceptionally rare and typically present unilaterally.
  • Bilateral CPA lipomas are exceedingly uncommon, posing diagnostic challenges.

Observation:

  • A 36-year-old male patient presented with bilateral aural fullness.
  • Magnetic resonance imaging (MRI) revealed bilateral CPA masses with extension into the internal auditory canals.

Findings:

  • The lesions were hyperintense on T1- and T2-weighted MRI sequences.
  • Facial and vestibulo-cochlear nerves, vascular structures, and septa were visualized within the masses on T2-weighted 3D FIESTA sequences.
  • No enhancement was observed after gadolinium administration.
