Summary

Hepatitis E virus (HEV) can cause acute liver disease. In a rare case, a healthy individual developed chronic liver disease and significant fibrosis from HEV infection.

Background:

  • Hepatitis E virus (HEV) comprises four genotypes, with Genotypes 1 and 2 causing outbreaks in Asia and Africa, and Genotypes 3 and 4 leading to sporadic hepatitis in Europe and North America.
  • HEV infection is typically self-limiting, but can progress to chronic liver disease, primarily in immunocompromised individuals.

Observation:

  • A healthy, immunocompetent male presented with acute Hepatitis E virus infection.
  • The patient experienced a rapid progression of the disease.

Findings:

  • Within 1.5 years, the acute HEV hepatitis evolved into chronic liver disease.
  • Liver biopsy revealed significant fibrosis, graded as III/IV.

Implications:

  • This case highlights the potential for HEV to cause severe chronic liver disease even in immunocompetent individuals.
  • Further research is needed to understand the mechanisms driving HEV chronicity in non-immunocompromised hosts.
