  • Radiological differentials included nerve sheath tumor and meningioma.

Findings:

  • Histopathology confirmed a cavernous hemangioma, a rare cause of purely extradural spinal masses.
  • The lesion was ovoid, T2 hyperintense, and lacked connection to neural structures or intervertebral discs.
  • Complete surgical excision was achieved via hemilaminectomy with preserved planes.

Implications:

  • Purely extradural cavernous hemangiomas should be considered in the differential diagnosis of spinal epidural soft tissue masses.
  • Specific imaging findings can help differentiate hemangiomas from more common spinal pathologies.
  • Understanding the rarity and presentation of these lesions aids in surgical planning and patient outcomes.
