Summary

For early-stage endometrial cancer, lymph node sampling may not be necessary. Histological grade and myometrial invasion accurately predict recurrence and survival, suggesting a revised approach to surgical management.

Area of Science:

  • Gynecologic Oncology
  • Surgical Oncology
  • Pathology

Background:

  • Standard endometrial cancer treatment includes lymph node sampling during hysterectomy, which increases surgical time and morbidity.
  • The necessity of lymphadenectomy in early-stage endometrial cancer is debated, especially when predictive factors are available.

Purpose of the Study:

  • To evaluate the impact of lymph node sampling on recurrence and survival rates in stage I endometrial cancer.
  • To determine if histological grade and myometrial invasion can serve as accurate predictors, potentially obviating the need for lymphadenectomy.

Main Methods:

  • Retrospective review of 247 women with stage I endometrial carcinoma.
  • Comparison of recurrence and survival rates between patients who underwent lymph node sampling (LNS) and those who did not (TAH+BSO only).

  • Analysis of myometrial invasion and histological grade as predictive factors.
  • Main Results:

    • Recurrence rates were similar: 13.5% with LNS vs. 12.9% without.
    • 5-year survival rates were comparable: 75.7% with LNS vs. 74.2% without.
    • Myometrial invasion and histological grade were identified as highly accurate predictors.

    Conclusions:

    • Lymph node sampling may not be essential for all stage I endometrial cancer patients.
    • Histological grade and myometrial invasion are reliable predictors, supporting a revised management algorithm.
    • Consideration should be given to omitting lymphadenectomy in high-grade cases based on predictive factors.
