Summary

This study found that some children with selective mutism also have epilepsy and abnormal EEG findings. While seizures were manageable, the selective mutism often persisted, suggesting a potential link or co-morbidity.

Area of Science:

  • Child Neurology
  • Child Psychiatry
  • Neurophysiology

Background:

  • Selective mutism is a childhood anxiety disorder characterized by a consistent failure to speak in specific social situations.
  • Electroencephalography (EEG) is typically not indicated for the evaluation of selective mutism.
  • Epileptic discharges are not usually considered part of the selective mutism clinical presentation.

Purpose of the Study:

  • To investigate the association between selective mutism and electroencephalographic (EEG) abnormalities.
  • To explore the potential co-morbidity or underlying organic brain disorder in children with selective mutism and epilepsy.

Main Methods:

  • Case report describing 6 children with selective mutism.
  • Review of medical history including epilepsy.

  • Analysis of interictal or subclinical electroencephalography (EEG) recordings.
  • Main Results:

    • Six children with selective mutism presented with a history of epilepsy and abnormal EEG findings.
    • Two children had benign epilepsy of childhood with centro-temporal spikes.
    • Seizures were controlled with medication in all cases, but selective mutism resolved in only one child.

    Conclusions:

    • Selective mutism may be associated with EEG abnormalities, suggesting a potential co-morbidity or role in pathogenesis.
    • Selective mutism could be a symptom of a more complex organic brain disorder.
    • These findings suggest that selective mutism should be considered among psychiatric disorders associated with EEG abnormalities.
