  • Arthroscopic assessment of anteromedial (AM), intermediate (IM), and posterolateral (PL) graft bundles for tension, damage, and synovial coverage.
  • Main Results:

    • Triple-bundle grafts exhibited a fan-out shape, mimicking natural ACL.
    • High graft tension was observed: 93% AM, 90% IM, 88% PL.
    • No significant rupture in AM/IM grafts; 10% of PL grafts showed complete/substantial rupture near the femoral aperture.
    • Synovial coverage was good in most grafts, but 41% of PL grafts had incomplete coverage.

    Conclusions:

    • The morphology of triple-bundle ACL grafts closely resembles the natural ACL.
    • Posterolateral (PL) grafts demonstrated a higher incidence of rupture and incomplete synovial coverage compared to AM and IM grafts.
    • Further investigation into PL graft integrity in triple-bundle ACL reconstruction is warranted.
