A rare isolated cleft in the anterior mitral leaflet caused severe mitral regurgitation. Minimally invasive repair using direct closure and annuloplasty proved successful in an adult patient.

- Annuloplasty was completed using a 30-mm Carpenter-Edwards Physio Ring.
Implications:
- Minimally invasive mitral valve repair is a viable option for severe mitral regurgitation due to isolated clefts.
- Video-assisted thoracic surgery offers excellent visualization for complex mitral valve repairs.
