Background:

Optic nerve MRI typically shows restricted diffusion in infarction and inflammation.
Infiltrative neoplasm of the optic nerve is not commonly associated with restricted diffusion on MRI.

Observation:

A patient with non-Hodgkin B-cell lymphoma developed acute optic neuropathy.
MRI revealed restricted diffusion in the intraorbital optic nerve, despite no brain or meningeal involvement.
Visual function did not improve with standard treatments, and restricted diffusion persisted for 4 months.

Findings:

Persistent restricted diffusion in the optic nerve on MRI in a patient with lymphomatous optic neuropathy.
This finding is atypical for infarction and may indicate the dense cellularity of the neoplasm.

Implications:

Restricted diffusion on MRI may be an indicator of infiltrative lymphomatous optic neuropathy.
This imaging finding could aid in the diagnosis and management of optic nerve lymphoma.
Further research is needed to understand the significance of persistent restricted diffusion in neoplastic conditions.
