Background:

  • CD20 is a B-cell marker, typically used for B-lineage cell identification.
  • CD20-positive T-cell lymphomas are infrequent but recognized, with varied clinical and histoimmunochemical profiles.
  • Primary cutaneous CD20-positive T-cell lymphoma is exceptionally rare, with only eight prior reports.

Observation:

  • This study presents two new cases of primary cutaneous CD20-positive T-cell lymphoma.
  • The cases detail clinical, histological, and immunohistochemical features.
  • CD20 expression has been noted in a subset of normal T-cells, often co-expressing CD8 and g/d.

Findings:

  • Two theories explain CD20 expression in neoplastic T-cells: origin from a CD20-positive T-cell subset or CD20 as an activation marker.
  • The specific subset of T-cells expressing CD20 also more frequently expresses CD8 and g/d.

Implications:

  • Further research is needed to elucidate the prognostic implications of CD20 positivity in T-cell lymphomas.
