Summary

Degenerative brain diseases can cause spatial neglect by disrupting large-scale attention networks, not just individual brain areas. Different patterns of network damage lead to varied neglect syndromes, impacting spatial working memory.

  • To explore how damage to specific neural subsystems of spatial attention leads to different neglect presentations.

Main Methods:

  • Studied five patients with neurodegenerative diseases: corticobasal degeneration (CBD), Posterior Cortical Atrophy (PCA), and frontotemporal dementia (FTD).
  • Analyzed lesion patterns and resulting neglect symptoms, including allocentric, egocentric, and motor-executive neglect.

Main Results:

  • Corticobasal degeneration (CBD) with left parietoccipital atrophy caused right allocentric neglect.
  • Posterior Cortical Atrophy (PCA) in right posterior regions led to left egocentric extrapersonal neglect.
  • Frontotemporal dementia (FTD) with right frontotemporal atrophy resulted in left motor-executive neglect; all patients had impaired spatial working memory.

Conclusions:

  • Left neglect is more common than right neglect in degenerative pathologies.
  • Damage to distinct neural substrates underlies different types of neglect.
  • Findings support models of spatial attention involving both lateralized and nonlateralized attention disorders.
