Summary
Transarterial embolization effectively treated high-flow priapism in an 18-year-old male following perineal trauma. This minimally invasive procedure resolved the arteriovenous fistula, restoring normal erectile function.
Area of Science:
- Urology
- Interventional Radiology
- Vascular Surgery
Background:
- Post-traumatic priapism is a rare but serious complication of perineal trauma.
- High-flow priapism often results from an arteriovenous fistula (AVF) involving the internal pudendal artery.
Observation:
- An 18-year-old male presented with high-flow priapism after blunt perineal trauma.
- Color Doppler ultrasound and selective arteriography confirmed an AVF originating from the left internal iliac artery branches.
Findings:
- Transarterial embolization using microcoils successfully occluded the AVF.
- Immediate improvement in detumescence and fistula closure were observed post-embolization.
- Normal erectile function was maintained at 6 and 12-week follow-ups.
Implications:
- Transarterial embolization is a safe and effective treatment for high-flow priapism.
- This minimally invasive approach offers a viable alternative to surgical intervention.
- Prompt management can preserve erectile function in patients with post-traumatic priapism.
