Background:

  • Systemic sclerosis (SSc) is an autoimmune disease with complex etiology.
  • The role of environmental factors, such as cigarette smoking, in SSc development is not fully understood.

Purpose of the Study:

  • To investigate the association between cigarette smoking and the risk of developing systemic sclerosis (SSc).
  • To explore potential links between smoking and specific SSc autoantibodies.

Main Methods:

  • A case-control study involving 1,379 SSc patients from registries and cohorts.
  • Smoking history was collected via chart review or interviews.
  • SSc patients were matched 2:1 with control subjects from the Behavioral Risk Factor Surveillance System based on age, sex, ethnicity, and residence.

Main Results:

  • No significant difference in smoking behavior was found between SSc patients and control subjects (OR 1.020, P=0.842).
  • Anti-topoisomerase I antibody-positive SSc patients were more likely to be never smokers (OR 0.648, P=0.049).
  • No association was observed between smoking and anticentromere or anti-RNA polymerase III antibodies.

Conclusions:

  • Cigarette smoking does not appear to increase the risk of developing systemic sclerosis.
  • Smoking may influence SSc disease severity and autoantibody profiles.
