Background:

  • Cognitive impairment is a significant challenge in multiple sclerosis (MS) management.
  • Existing cognitive tests may not be suitable for routine clinical use in MS patients.
  • A brief, computerized cognitive test battery is needed for efficient clinical assessment.

Purpose of the Study:

  • To investigate the validity and utility of the Cognitive Drug Research (CDR) computerized cognitive test system.
  • To longitudinally assess cognitive performance in patients with relapsing-remitting multiple sclerosis (RRMS).

Main Methods:

  • Forty-three mildly disabled RRMS patients were assessed using the CDR System, Digit Symbol Substitution Test (DSST), and Paced Auditory Serial Addition Test (PASAT).
  • Disability was measured using the Multiple Sclerosis Functional Composite (MSFC) and Expanded Disability Status Scale (EDSS).
  • Cognitive performance was evaluated longitudinally using composite scores from the CDR System, including measures of attention and memory.

Main Results:

  • CDR System composite scores demonstrated good test-retest reliability, with minor exceptions for working and episodic memory.
  • CDR measures strongly correlated with established cognitive tests (DSST, PASAT) and disability scales (EDSS, MSFC).
  • Patients showed significant impairments in visual information processing speed, attention, and complex information processing speed compared to healthy controls.

Conclusions:

  • The CDR System shows preliminary validity and utility for assessing cognition in MS patients.
  • Most CDR measures exhibited adequate psychometric properties and correlated with disability and other cognitive assessments.
  • Further validation is recommended for specific memory measures within the CDR System.
