Summary

Colorectal polyps can originate from mesenchymal tissue. A rare case of a benign mucosal Schwann cell hamartoma was identified, distinct from schwannoma.

Background:

  • Colorectal polyps of mesenchymal origin are uncommon gastrointestinal (GI) lesions.
  • Their detection is increasing due to widespread colonoscopy screening.

Observation:

  • A case of a small colonic polyp presented as an intramucosal diffuse spindle cell proliferation.
  • The lesion exhibited benign cytology and strong, diffuse S-100 protein immunoreactivity.
  • A pure Schwann cell phenotype was identified.

Findings:

  • Morphological, immunohistochemical, and clinical evaluations differentiated this lesion from other stromal colonic lesions.
  • The findings distinguished it from schwannoma, a rare benign nerve sheath tumor in the GI tract.

Implications:

  • This case supports the recent proposal to classify this lesion as a mucosal Schwann cell hamartoma.
  • Accurate diagnosis is crucial for differentiating this benign hamartoma from other mesenchymal tumors of the colon.
