Summary

Younger patients with unexplained chest pain and shortness of breath may have drug abuse issues. Early screening for substance use is crucial for diagnosing conditions like dilated cardiomyopathy.

  • Dyspnoea and chest pain are common presenting symptoms in primary care.
  • Misdiagnosis can lead to delayed treatment and adverse outcomes.

Observation:

  • A young patient presented with dyspnoea and chest pain, initially diagnosed as pneumonia.
  • The patient later developed generalized oedema and was hospitalized.
  • Echocardiography revealed dilated cardiomyopathy.

Findings:

  • Diagnostic delay occurred due to failure to inquire about potential drug abuse.
  • Drug abuse can contribute to the development of cardiac conditions.

Implications:

  • Clinicians should consider substance abuse screening in younger patients presenting with atypical cardiopulmonary symptoms.
