Summary

Tadalafil treatment reduced endothelial damage markers in men with erectile dysfunction. However, these positive effects on endothelial function disappeared six months after discontinuing the medication.

Background:

  • Phosphodiesterase V inhibitors are known to improve endothelial dysfunction in erectile dysfunction (ED).
  • Limited research exists on the duration of endothelial effects after discontinuing these medications.

Purpose of the Study:

  • To evaluate serum concentrations of apoptotic endothelial microparticles (EMPa) as a marker of endothelial damage.
  • To assess the persistence of tadalafil's endothelial benefits after discontinuation in patients with arterial ED.

Main Methods:

  • 50 patients with arterial ED received tadalafil 5 mg daily for 90 days.
  • Clinical (IIEF-5), instrumental (Doppler), and flow-cytometric (serum EMPa) analyses were performed at baseline, 1 week, 3 months, and 6 months post-discontinuation.
  • EMPa were defined as CD45(neg)/CD144(pos)/annexinV(pos) events.

Main Results:

  • Patients with arterial ED had higher baseline serum EMPa concentrations than healthy controls.
  • Tadalafil administration significantly reduced EMPa levels at 1 week post-treatment.
  • This reduction was maintained, albeit diminished, at 3 months but completely lost by 6 months post-discontinuation.

Conclusions:

  • Tadalafil administration effectively decreases endothelial damage markers in patients with arterial ED.
  • The beneficial effects of tadalafil on endothelial dysfunction are temporary and resolve within 6 months of cessation.
