Muscles of the Shoulder

The muscles surrounding the shoulder girdle, including the clavicle and scapula, primarily stabilize the scapula. This stable base allows other muscles to move the humerus effectively. Scapular movements often mirror those of the humerus and extend its range of motion. For instance, raising the arm above the head would not be feasible without simultaneous upward rotation of the scapula.

Background:

  • Fracture dislocation of the shoulder is a common injury from road traffic accidents.
  • Peripheral nerve blocks offer rapid, prolonged analgesia for pain and surgical management.
  • Interscalene block is not widely reported as a primary anesthetic for shoulder surgery in our region.

Observation:

  • A case of open reduction and internal fixation for left humeral surgical neck fracture dislocation was managed.
  • Interscalene brachial plexus block was performed using a nerve stimulator and a local anesthetic mixture.
  • The anesthetic involved 40 ml of 0.25% bupivacaine and 1.0% lidocaine with epinephrine.

Findings:

  • Successful surgical anesthesia was achieved 18 minutes after block placement.
  • The surgery, lasting 70 minutes, proceeded without any complications.
  • The interscalene block provided adequate anesthesia for the orthopedic procedure.

Implications:

  • This case demonstrates the successful use of interscalene block as a primary anesthetic technique for shoulder fracture dislocations.
  • This approach may serve as a viable alternative to general anesthesia, potentially mitigating its risks.
  • Further studies could explore the broader application of interscalene blocks in managing shoulder injuries.
