Area of Science:

  • Surgical innovation
  • Minimally invasive surgery
  • Endoscopic techniques

Background:

  • Robotic thyroidectomy adoption in the US lacked extensive preclinical data.
  • Existing robotic thyroidectomy techniques have varying dissection extents.

Purpose of the Study:

  • To systematically investigate a novel remote access robotic thyroidectomy via a facelift incision.
  • To compare the dissection extent of the facelift approach with a transaxillary robotic technique.

Main Methods:

  • Preclinical investigation using surgical simulation and morphometric analysis in human cadavers.
  • Development of a reproducible surgical protocol and dissection algorithm for facelift robotic thyroidectomy.
  • Quantification of dissection area for both facelift and transaxillary robotic approaches.

Main Results:

  • A reproducible protocol for robotic facelift thyroidectomy was established.
  • The mean dissection area for robotic facelift thyroidectomy (39.2 cm²) was 38.3% smaller than for robotic axillary thyroidectomy (63.5 cm²).
  • Statistical significance was achieved (P < .0001).

Conclusions:

  • A novel, reproducible robotic facelift thyroidectomy technique has been developed and refined.
  • This approach requires significantly less dissection compared to the transaxillary method.
  • Cautious clinical implementation for safety and feasibility is warranted.
