Summary

Sulcus transscleral intraocular lens suture fixation is a safe and effective technique for eyes with posterior capsule defects. This method offers improved visual acuity and minimal complications for secondary lens fixation.

Background:

  • Posterior capsule defects and insufficient zonular support pose challenges in cataract surgery.
  • Secondary intraocular lens (IOL) fixation is often required in such cases.

Purpose of the Study:

  • To evaluate the clinical efficiency and safety of sulcus transscleral IOL suture fixation.
  • To assess this technique in eyes with posterior capsule defects or insufficient zonular support.

Main Methods:

  • A cohort of 39 eyes with severe posterior capsule defects or zonular damage was studied.
  • Sulcus transscleral IOL suture fixation was performed using a small scleral tunnel incision and a puncture needle-guided external approach.
  • A "one- or two-point fixation" method was employed.

Main Results:

  • The mean follow-up period was 12.1 months.
  • Best-corrected visual acuity of 20/40 or better was achieved in 87.17% of eyes (34 eyes).
  • No intraoperative or postoperative complications like hemorrhage, retinal detachment, or IOL tilt/decentration were observed.

  • Sulcus transscleral IOL suture fixation is easy to master and reduces operative time.
  • The technique is effective for primary IOL fixation in eyes with compromised posterior capsules or zonules.
  • It is a safe and efficient option for secondary IOL fixation.
