Summary

Blacks in urban areas were less likely to receive tissue plasminogen activator (tPA) for ischemic stroke. This disparity was linked to contraindications, delayed symptom presentation, and stroke severity, not eligibility. Culturally relevant education is needed.

Background:

  • Racial disparities in intravenous tissue plasminogen activator (tPA) use for acute ischemic stroke have been previously documented.
  • Understanding these disparities within specific populations is crucial for targeted interventions.

Purpose of the Study:

  • To investigate the association between race and tPA treatment for stroke.
  • Focus on a predominantly Black urban population to identify specific contributing factors.

Main Methods:

  • Systematic chart review of consecutive ischemic stroke patients.
  • Data collected from all 7 acute care hospitals in Washington D.C.
  • Study period: February 1, 2008, to January 31, 2009.

Main Results:

  • In a cohort of 1044 ischemic stroke patients (74% Black), only 5% received tPA.
  • Black patients were less likely to receive tPA (3% vs. 10% for Whites), present within 3 hours (13% vs. 21%), and be tPA-eligible (5% vs. 13%).
  • Among those presenting within 3 hours, Black patients were less likely to be treated (27% vs. 46%), but eligibility-based treatment rates were similar (70% vs. 76%).

Conclusions:

  • In this urban Black population, Black patients received tPA less often due to contraindications, delayed presentation, and stroke severity.
  • Effective interventions require culturally tailored education addressing population-specific barriers to tPA treatment.
