Summary

Polyethylene wear in reverse shoulder replacements primarily occurs at the rim, especially when screws contact the liner. This rim damage can accelerate wear and potentially loosen the glenoid component.

  • All retrieved components showed rim wear on the polyethylene humeral cup, with depths ranging from 0.1 to 4.7 mm.
  • No correlation was found between scapular notching and rim wear.
  • Extensive rim wear was associated with contact between the inferior screw and the liner; metal-on-metal wear was also observed in one case.

Conclusions:

  • Rim damage is the primary mode of polyethylene wear in retrieved RTSA specimens.
  • Direct contact between the humeral component and inferior glenoid screws is a significant concern.
  • This contact may accelerate ultra-high-molecular-weight polyethylene wear and lead to glenoid component loosening.
