Radioisotopes for metastatic bone pain

Radioisotopes offer a small benefit in relieving metastatic bone pain, but can cause significant side effects like low blood cell counts. Further research is needed to compare different radioisotopes and doses.

- Current treatments for bone metastasis pain include analgesia, radiotherapy, surgery, chemotherapy, hormone therapy, radioisotopes, and bisphosphonates.
Purpose of the Study:
- To evaluate the efficacy and safety of radioisotopes in managing metastatic bone pain.
- To assess the impact of radioisotopes on reducing complications and improving survival in patients with bone metastases.
Main Methods:
- A systematic review of randomized controlled trials (RCTs) was conducted, searching databases including MEDLINE, EMBASE, and CENTRAL up to October 2010.
- Studies included patients with metastatic bone pain treated with radioisotopes compared to placebo or other radioisotopes.
- Risk of bias was assessed, and data were analyzed using random-effects models with intention-to-treat sensitivity analysis.
Main Results:
- Radioisotopes showed a small benefit in complete pain relief (NNT=5) and complete/partial relief (NNT=4) in the short to medium term.
- No conclusive evidence was found that radioisotopes reduce the need for analgesics compared to placebo.
- Significant adverse effects, including leucocytopenia and thrombocytopenia (NNH=13), were associated with radioisotope administration.
Conclusions:
- Radioisotopes may offer pain reduction for bone metastases over one to six months without increasing analgesic use.
- However, severe adverse effects such as leucocytopenia and thrombocytopenia are common.
- Limited data exist for comparing different radioisotopes (e.g., Strontium-89 vs. Samarium-153) and doses.
