Summary

A new postal audit using Thermoluminescent Dosimeters (TLD) effectively verifies absorbed doses in small photon beams used in Intensity Modulated Radiotherapy (IMRT) and Stereotactic Radiosurgery (SRS). This method ensures accurate dose delivery for advanced radiation therapy techniques.

Area of Science:

  • Medical Physics
  • Radiation Oncology
  • Dosimetry

Background:

  • Advanced radiotherapy techniques like IMRT and SRS increasingly utilize small radiation beams (<4 cm).
  • Accurate dose measurement in small beams presents unique challenges compared to larger beams.
  • Quality assurance for small beam dosimetry is critical for patient safety and treatment efficacy.

Purpose of the Study:

  • To develop and validate a postal audit system for checking absorbed doses in small photon beams.
  • To assess the accuracy and suitability of Thermoluminescent Dosimeters (TLD) for this purpose.
  • To ensure reliable dose verification in clinical settings employing advanced radiotherapy.

Main Methods:

  • A postal audit system using small TLD-100 chips (1x1x1 mm³) within a PMMA phantom was designed.

  • The system was mailed to 14 centers for irradiation with 1 cm and 3 cm side/diameter beams at a prescribed dose of 1.5 Gy at 10 cm depth.
  • System properties were validated through experimental measurements and Monte Carlo simulations prior to the audit.
  • Main Results:

    • The audit achieved dose deviations below 5% in 69% of centers for 1x1 cm² beams and 64% for 3x3 cm² beams.
    • Investigated deviations exceeding 5% led to necessary corrections, improving overall accuracy.
    • The system demonstrated a high level of agreement between prescribed and measured doses.

    Conclusions:

    • The developed TLD postal audit system is suitable for verifying absorbed doses in small photon beams.
    • The audit provides accurate dose verification with an overall accuracy of 2.9% (1 s).
    • This method enhances quality assurance for advanced radiation therapy techniques involving small beams.
