Background:

  • Tissue engineering offers significant potential for bone defect regeneration.
  • A patient with a proximal femur cyst received impaction bone grafting with autologous bone marrow.
  • Recurrence of the cyst necessitated a total hip replacement, allowing for analysis of the engineered bone.

Observation:

  • Ex vivo analysis of the retrieved femoral head was performed using micro-computed tomography, histology, and mechanical testing.
  • The engineered bone showed a trabecular structure with nonincorporated graft islands.
  • The graft was denser than native bone but had comparable strength.

Findings:

  • The cyst material had infiltrated the bone graft.
  • An increased number of osteoclasts were observed, indicating bone resorption.
  • While some remodeling occurred, the bone marrow aspirate had a low concentration of osteoprogenitor cells.

Implications:

  • Insufficient cellularity in the bone graft contributed to construct failure.
  • The osteoclastic activity associated with cyst recurrence was not overcome by the available cells.
  • Augmenting bone grafts with concentrated or cultured osteoprogenitor cells is recommended for improved outcomes.
