Area of Science:

  • Cardiology
  • Transplant Surgery
  • Electrophysiology

Background:

  • Heart block is a known complication after cardiac transplantation.
  • Diagnosis typically relies on surface electrocardiogram (ECG).

Observation:

  • A 24-year-old male with a prior cardiac transplant presented with symptoms suggestive of complete atrioventricular nodal block.
  • Surface ECG showed junctional escape rhythm.

Findings:

  • During pacemaker implantation, sinus rhythm was confirmed in the recipient atrium.
  • A block was identified at the atrioatrial anastomosis.
  • Sinus arrest was observed in the donor atrium, with intact atrioventricular nodal conduction.

Implications:

  • This case demonstrates an atypical presentation of sinus arrest post-heart transplantation.
  • It underscores the need for detailed electrophysiological assessment beyond standard ECG in complex transplant cases.
  • Understanding these rare arrhythmias is crucial for effective management of cardiac transplant recipients.
