  • Pancreas transplantation (PTx) is the definitive treatment for type 1 diabetes.
  • Advancements in diabetes care have increased the number of older PTx candidates.
  • Evaluating outcomes in older PTx recipients is crucial.

Purpose of the Study:

  • To assess the feasibility and outcomes of PTx in patients aged 50 years and older.
  • To compare surgical complications, rejection rates, infections, and survival in older versus younger PTx recipients.

Main Methods:

  • Retrospective review of 136 consecutive PTx patients (1996-2010).
  • 17 patients were aged ≥50 years.
  • Analysis of demographics, surgical complications, acute rejection (AR), infections, and survival.

Main Results:

  • Demographics were similar between age groups, except for age.
  • Comparable rates of major and minor surgical complications were observed.
  • Older recipients had significantly lower 1-year and overall AR rates.
  • Non-surgical infections and patient/graft survival were similar between groups.

Conclusions:

  • Pancreas transplantation is feasible in patients aged 50 and older.
  • Older PTx candidates experience low surgical morbidity, infection rates, and acute rejection.
  • Age should not be a contraindication for pancreas transplantation in select type 1 diabetes patients.
