Background:

Purpose of the Study:

  • To evaluate the feasibility of diffusion-weighted magnetic resonance (MR) imaging (DWI) for assessing perianal fistula activity.
  • To determine if DWI can differentiate between active and inactive perianal fistulas.

Main Methods:

  • Retrospective analysis of 28 MR examinations in 24 patients with suspected perianal fistulas.
  • Diffusion-weighted imaging (DWI) performed at 1.5 T with b-values of 0 and 1000 s/mm(2).
  • Apparent diffusion coefficient (ADC) values measured and compared between groups with positive (PIA) and negative (NIA) inflammation activity.

Main Results:

  • Significantly lower ADC values were observed in the PIA group (0.908±0.171) compared to the NIA group (1.124±0.244) (P=0.0019).
  • Receiver Operating Characteristic (ROC) analysis showed an area under the curve of 0.749.
  • An optimal ADC cut-off of 1.109 demonstrated high sensitivity (95.7%) and negative predictive value (90%).

Conclusions:

  • Diffusion-weighted imaging (DWI) is a feasible technique for evaluating perianal fistula activity.
  • ADC values derived from DWI show potential in distinguishing active from inactive fistulas.
  • Further prospective studies are needed to confirm the diagnostic performance of DWI.
