Patients attending emergency departments (EDs) and walk-in centres (WiCs) often expect different services than what is provided. Understanding patient expectations is key to improving unscheduled care delivery and resource allocation.

Background:

  • Unscheduled care services, including emergency departments (EDs) and walk-in centres (WiCs), face challenges in managing increasing demand.
  • Developing effective unscheduled care strategies requires understanding patient needs and expectations in relation to service provision.

Purpose of the Study:

  • To identify the appropriate service provider attendees of EDs and WiCs in North East London.
  • To match patient expectations and choices with local service provision.

Main Methods:

  • Anonymous patient surveys and retrospective analysis of patient records were conducted.
  • A nurse consultant, general practitioner (GP), and pharmacist independently streamed patients based on presenting complaints.
  • Statistical analysis of agreement and a stakeholder focus group reviewed the findings.

Main Results:

  • Younger patients (mean age 35.6) were more frequent attendees.
  • Over 40% of patients could not obtain a convenient GP appointment, and over 40% of those already receiving treatment considered their condition an emergency.
  • Most patients expected to see a doctor, with significant variation in expectations for prescriptions and health advice between EDs and WiCs.

Conclusions:

  • Patient expectations regarding provider type and service received do not always align with the intended service model (e.g., nurse-led WiCs).
  • The plurality of unscheduled care services can lead to duplication and repeat attendance.
  • Service development and delivery can be informed by understanding patient decision-making uncertainties in accessing care.
