Area of Science:

  • Radiation Oncology
  • Medical Physics
  • Clinical Informatics

Background:

  • Intensity modulated radiation therapy (IMRT) is a common treatment for prostate cancer.
  • Developing optimal IMRT plans requires significant expertise and time.
  • A knowledge-based approach could streamline this process by leveraging existing treatment data.

Purpose of the Study:

  • To assess the feasibility of generating new prostate IMRT plans using a knowledge base of prior treatment plans.
  • To demonstrate that new plans can be created by matching new cases to similar historical plans.

Main Methods:

  • A database of 100 prostate IMRT plans was established.
  • An information-theoretic system and mutual information algorithm were used to match patient cases based on contour projections.

  • New treatment plans were generated using parameters from the most similar, clinically approved plan in the database.
  • Main Results:

    • New knowledge-based plans achieved comparable planning target volume coverage (within 2%) to original plans.
    • Dose sparing for the rectum and bladder in new plans was comparable to original plans.
    • Negative percentage differences in dose metrics indicate improved sparing for organs at risk in the new plans.

    Conclusions:

    • A knowledge-based approach can generate clinically acceptable, high-quality prostate IMRT plans.
    • This semi-automated method has the potential to enhance treatment planning efficiency.
    • The approach ensures the development of high-quality treatment plans for prostate cancer patients.
