Purpose of the Study:

  • To compare the utilization of radical nephrectomy (RN) versus partial nephrectomy (PN) in patients aged 75 years and older versus younger patients.
  • To analyze trends in surgical management of localized renal cell carcinoma (RCC) over a decade.

Main Methods:

  • Analysis of 18,045 localized RCC cases (≤4 cm) from the Surveillance, Epidemiology, and End Results registry (1998-2007).
  • Comparison of demographic and tumor characteristics between age cohorts (<75 vs. ≥75 years).
  • Multivariate logistic regression to determine predictors of radical nephrectomy for clinical Stage T1a disease.

Main Results:

  • Patients aged ≥75 years underwent radical nephrectomy (RN) more frequently (66%) than younger patients (59%).
  • For tumors ≤2 cm, 51% of elderly patients had RN compared to 41% of younger patients.

  • Elderly patients with small, localized renal masses are more likely to be treated with radical nephrectomy (RN).
  • Further research is warranted to explore the medical implications of increased radical surgery rates in the geriatric population.
