Summary

Isopropyl alcohol (IPA) intoxication presents uniquely in alcoholics as stupor and in hidden addicts as encephalopathy. Recognizing these distinct neurologic signs is crucial for diagnosis and treatment.

Area of Science:

  • Toxicology
  • Neurology
  • Internal Medicine

Background:

  • Isopropyl alcohol (IPA) is a common solvent with potential for abuse.
  • Alcoholic patients may develop neurologic symptoms from IPA intoxication when ethanol is unavailable.
  • Distinguishing IPA intoxication from other alcohol-related disorders is clinically significant.

Observation:

  • Three patients presented with distinct neurologic signs attributed to isopropyl alcohol (IPA) intoxication.
  • One patient, a known alcoholic, exhibited apathy, confusion, ataxia, and hyperreflexia over several weeks, denying other substance use.
  • Two other patients were admitted in stuporous states following significant IPA ingestion.

Findings:

  • IPA intoxication can manifest in two primary ways: stupor in known alcoholics and encephalopathy of unknown origin in individuals concealing their addiction.
  • Clinical and laboratory findings differ among intoxications with ethanol, methanol, IPA, and ethylene glycol.

Implications:

  • Awareness of these varied presentations is vital for accurate diagnosis of IPA abuse.
  • Prompt identification and management of IPA intoxication can prevent severe neurologic sequelae.
  • This highlights the importance of comprehensive substance abuse history and toxicology screening.
