Summary

Diffusion-weighted imaging (DWI) shows significantly lower apparent diffusion coefficient (ADC) values in brain death (BD) patients. This technique offers a 100% accurate, noninvasive method for diagnosing brain death.

Background:

  • Brain death (BD) diagnosis relies on clinical criteria.

  • Diffusion-weighted imaging (DWI) is an emerging technique in neuroimaging.

Purpose of the Study:

  • To determine the role of DWI in diagnosing brain death (BD).
  • To investigate the efficacy of DWI as a confirmatory test for BD.

Main Methods:

  • Prospective evaluation of 22 clinically diagnosed BD patients.
  • Acquisition of diffusion-weighted images (DWI), T2-weighted images, and fluid-attenuated inversion recovery (FLAIR).
  • Analysis of apparent diffusion coefficient (ADC) values in 13 neuroanatomical structures.

Main Results:

  • Significantly lower ADC values were observed in all neuroanatomical structures of BD patients compared to controls.
  • Threshold ADC values were established to differentiate between BD and control groups.
  • ADC cutoff values demonstrated 100% sensitivity, specificity, and correct classification rate.
