Summary

Resecting the cremaster muscle during Shouldice hernia repair significantly reduces recurrence rates. This prospective trial found no difference in complications but fewer indirect recurrent hernias when the muscle was removed.

Area of Science:

  • Surgery
  • Surgical Techniques
  • Hernia Repair

Background:

  • The Shouldice repair is a common surgical technique for inguinal hernias.
  • The role of cremaster muscle resection in Shouldice repair outcomes is debated.

Purpose of the Study:

  • To evaluate the impact of cremaster muscle resection on recurrence rates and complications in Shouldice hernia repair.
  • To compare outcomes between Shouldice repairs with and without cremaster muscle resection.

Main Methods:

  • A prospective controlled trial comparing 237 Shouldice repairs with cremaster resection to 153 without.
  • Follow-up ranged from 12 to 26 months for both groups.
  • Outcomes assessed included postoperative complications and hernia recurrence.

Related Experiment Videos

Main Results:

  • No significant differences in postoperative complications were observed between the groups.
  • The group without cremaster resection had a significantly higher rate of indirect recurrent hernias (p < 0.05).
  • Resection of the cremaster muscle was associated with a lower incidence of recurrent hernias, particularly indirect types.

Conclusions:

  • Cremaster muscle resection is an essential component of Shouldice hernia repair.
  • Resection of the cremaster muscle significantly reduces the risk of hernia recurrence.
  • The technique of Shouldice repair is influenced by the inclusion or exclusion of cremaster muscle resection.
