Summary

Post-stroke fatigue is a significant predictor of suicidal ideation (SI) in Chinese stroke survivors. Early identification and management of fatigue may help reduce suicidal risk in these patients.

Background:

  • Stroke is a leading cause of disability worldwide, often associated with significant psychological sequelae.
  • Post-stroke fatigue is a common and debilitating symptom impacting quality of life.
  • The relationship between fatigue and suicidal ideation (SI) in stroke patients requires further investigation.

Purpose of the Study:

  • To evaluate the association between post-stroke fatigue and suicidal ideation (SI) in Chinese patients experiencing their first or recurrent stroke.
  • To identify fatigue as a potential modifiable risk factor for SI in stroke survivors.

Main Methods:

  • A cross-sectional survey was conducted with 595 patients admitted to an acute stroke unit in Hong Kong.
  • Fatigue Severity Scale (FSS) and Geriatric Mental State Examination were used to assess fatigue and SI, respectively, at 3 months post-stroke.
  • Logistic regression analysis adjusted for confounders including age, sex, depression, and stroke severity (NIHSS).

Main Results:

  • 12.8% of patients reported SI.
  • Patients with SI had significantly higher Fatigue Severity Scale scores compared to those without SI (4.0±1.4 vs 2.8±1.2).
  • Fatigue Severity Scale score was a significant independent predictor of SI (odds ratio=1.5), even after adjusting for confounders.

Conclusions:

  • Post-stroke fatigue is strongly associated with suicidal ideation in Chinese stroke patients.
  • Clinicians should be vigilant in assessing and managing fatigue in stroke survivors.
  • Early intervention for fatigue may play a crucial role in mitigating suicidal risk.
